New Patient Registration


The doctors welcome new patients who live within our practice area. Patients living within our catchment area may register with us when the practice list is open.

The practice does not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition.

To register with the practice, please bring a form of ID i.e. passport or drivers licence along with your medical card (if you have one) or complete a patient registration form which can be obtained at reception. Please put as much information on it as possible, this speeds up the registration process. Your notes from your previous practice will then be requested to be sent here. This can take six weeks or longer. 

Please ensure that you have at least 30 days supply of any regular medication you are taking prior to leaving your current practice to avoid being without your medication. 

New patients registering with the practice will be offered a health check within the first three months of registering.

If you wish to register with the practice, please click on the link below, complete the form, print it off, sign it and return it to the surgery.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
