We would like to know how we can improve our service to you and how you perceive our surgery and staff.
To help us with this, we are setting up a virtual patient representation group so that you can have your say. We will ask the members of this representative group some questions from time to time, such as what you think about our opening times or the quality of the care or service you received. We will contact you via email and keep our surveys succinct so it shouldn’t take too much of your time.
We aim to gather around a hundred patients from as broad a spectrum as possible to get a truly representative sample. We need young people, workers, retirees, people with long term conditions and people from non-British ethnic groups.
The group holds 4 meetings a year; each lasts about an hour and a half. Our next meeting will be held on Tuesday 25th July 2017. It is fully supported by the GP partners, one of whom attends every meeting.

PATIENT PARTICIPATION GROUP CONSTITUTION
1) Name: The name of the group shall be Dr RJN Baker & Partners Patient Reference Group
The objectives of the Group shall be to represent the patients of the Practice, to gather and collate opinion from patients via a range of sources, and to comment and offer opinion on these views to representatives of the practice, to encourage development and quality of health promotion and health care services; to achieve this aim by liaising with the doctors and staff, other community health workers, Health Authorities and other persons or organisations concerned with health care.
The Group shall be non-party in politics and non-sectarian in religion, and shall be formed using the best principles of equality and diversity. The Group and the practice will make every effort to ensure that the constituent elements of the Group are representative of the practice patient demographic, with a range of patient interests represented.
The Group shall have power to affiliate to the National Association of Patient Participation Groups and to other organisations with similar objectives.
This shall be open to any patient of the Practice. Removal of a patient from the practice list, for whatever reason, will disqualify continuing membership of the group. The practice and the Group may approach individual patients for potential membership.
The group will meet at least 4 times a year.
The chair will be elected annually by members of the group and in the chair’s absence a floating chair will be voted in on the night.
The practice will ask the group for feedback on service development and provision.
Minutes shall be kept and the Secretary shall enter a record of all proceedings and resolutions.
Notices of meetings, minutes of meetings and information on the group’s activities will be displayed in practice waiting room and website.
This constitution was adopted as the Constitution of the Drs Baker Swamy Siddiqui PRG at a meeting of the Committee at Rushbottom Lane Surgery, 91 Rushbottom Lane, Benfleet Essex SS7 on 7th July 2015
